1. Highway 113 north towards Woodland, go approximately 1.5 miles.
2. Exit Russell Boulevard, turn right (East).
3. Proceed two traffic lights, turn left (North) onto Anderson Road.
4. Our clinic is located approximately 1 block on the left in the Davis Medical Center – 635 Anderson Road, Suite 1

Additional parking is located in the rear of the medical complex.

We are open Monday-Thursday 8:00 AM to 5:00 PM, Friday 8:00 AM to 3:00 PM.  Emergency on-call ophthalmology services are available 24-hours daily by calling (530) 756-5040 or going to Sutter Davis Hospital located at 2020 Sutter Place, Davis.

PLEASE REMEMBER TO BRING WITH YOU FOR YOUR APPOINTMENT:

  • INSURANCE:  please bring your cards with ID numbers.  If you are not the primary subscriber, please provide their full name and if necessary the subscriber’s date of birth (this data will not be recorded it is used to identify the correct account and obtain authorization for your visit).
  • GLASSES/CONTACTS:  please wear the glasses which provide the clearest vision and are most comfortable.  If you wear contact lenses, please bring the empty boxes so we can identify the brand and power.  Also, if wearing contact lenses, bring your glasses as well.  This will help make your visit go faster!
  • MEDICATIONS:  please provide us with a list your eye drops and other medications you are taking.  It is important for us to know what other medications you are taking to avoid interactions.
  • COPAYS:  many medical and insurance plans require we collect co-pays at the time of visit.  We accept cash, checks and major credit cards.  Due to contract agreements, co-pays will not be billed after January 1, 2009 and we must reschedule your visit.

MEDICARE PATIENTS:

  • Medicare does not provide for refraction (check for glasses) or for the glasses themselves.  If you wish to be checked for glasses a $35 fee will be due prior to your visit.

WHAT TO EXPECT DURING YOUR VISIT:

  • ARRIVAL:  if you are a new patient to our practice, welcome, and we ask you to arrive 10-15 minutes prior to your scheduled appointment to complete initial documentation (demographics and clinic policies).  Please check-in with Dyan or Bailey.
  • INSURANCE:  we make every attempt to obtain authorization prior to your visit from information we obtain when scheduling your appointment.  Please bring  your insurance card as we require a current insurance card on file at all times.  In the rare instance we cannot obtain authorization, we may request additional information from you prior to examination.
  • THE VISIT:  please schedule your appointment on a day you have at least an hour and a half for a complete dilated examination.  After the initial check for glasses is complete, dilation drops will be placed and you will be asked to wait for 15-30 minutes (you can visit our optical department at this time).  Following dilation, the retina and other internal structures will be examined.
  • AFTER THE VISIT:  your eyes will be dilated (pupils larger) which will make sunlight very bright and/or near objects blurry.  If you feel uncomfortable driving, please, please bring a driver along with you.  If you have a comfortable pair of sunglasses available, please bring them otherwise we can provide you with a disposable pair.  This is to ensure your safety and style!
